Trial Appointment Waiver Form

By signing below, I acknowledge that I understand and agree to the following terms:

• I understand that physical activity involves inherent risks of injury.

• I release the facility and its staff from any liability for injuries or damages.

• I understand that once class is in session, parents and siblings not enrolled in the class must stay in the reception area and avoid crowding doorways. Parents and siblings are not permitted to enter the dance floor to avoid distracting the dancers.

• I understand that photos and videos may be recorded during class and I give permission for my child to have their photo or video taken.

• I understand that a trial appointment does not guarantee enrollment. I understand that the purpose of the trial is for instructors and staff to make a class recommendation suitable to my child's age, skill level, and interest. I am also not obligated to enroll my child in a class if I am not satisfied with the class curriculum.

• I understand that my child may be dismissed from class if they are not wearing dance-appropriate clothing, being disruptive, or choosing not to participate.

• I understand that the trial deposit fee is non-refundable, even if my child does not finish class. I understand that the fee is also forfeited if I miss my appointment.
